Dear Establishment Owner:

This letter is to inform you that all 2010 Morgan County Tattoo/ Body Piercing Permits will expire on December 31, 2010. All establishments are required to obtain a new permit by January 1, 2011 in order to remain in operation.

A permit may be obtained by mail or in person. By mail, please complete and mail the enclosed application along with a self-addressed, stamped envelope with the appropriate fee (check or money order made payable to Morgan County Health Department). If you plan to pick your permit up in person, please bring the completed application with you.

NOTE: A fine of $25.00 will be assessed if applications are not postmarked by December 31, 2010, and $50.00 if not postmarked by January 15, 2010.
