Dear  Establishment  Owner:

 

This  letter  is  to  inform  you  that  all  Morgan  County  2008  Tattoo/ Body  Piercing  Permits  will  expire  on  December  31st of each year.   All  establishments  are  required  to  obtain  a  new  permit  by  January  1st   in  order  to  remain  in  operation.

 

A  permit  may  be  obtained  by  mail  or  in  person.   By  mail,  please  complete  and  mail  the  enclosed  application  along  with  a  self-addressed  stamped  envelope  with  the  appropriate  fee  (check  or  money  order  made  payable  to  Morgan  County  Health  Department).   Should  you  elect  to  pick  the  permit  up  in  person  please  bring  the  completed  application  with  you.

 

A  fine  of  $25.00  will  be  assessed  if  applications  are  not  postmarked  by  December  31st,  and  $50.00  if  not  postmarked  by  January  15th  of the year to be permited.
